APPLY TO BE ADOPTED​
Once your application is approved and you are matched with a sponsor, you will receive an email from Hearts of Patriots. 

IF YOU ARE ADOPTED AS A CAREGIVER
Sponsors are asked to provide up to $75 for a gift(s) just for you the Caregiver and a $100.00 grocery gift card to help with the Holiday meal. If you are adopted you will be asked to submit a WISH LIST to Hearts of Patriots of gift suggestions for yourself. The number of Caregivers adopted is based on the number of Sponsors we have to pair with the caregivers. (In some instances, a Sponsor will ask to adopt the caregiver and children. We will ask for a wish list for the children living at home under the age of 18 if we have a sponsor for your family.)
